I understand that my child(ren) and/or I may come into contact with foods while participating in activities hosted by Flour Power including (but not limited to) milk, eggs, wheat, peanuts, tree nuts, and shellfish. If my child(ren) and/or I are allergic to any of these foods or any others, I have specifically listed them on any applicable forms. If my child(ren) and/or I develop any new allergies after I fill out any applicable form but prior to my or my child(ren)’s participation in activities hosted by Flour Power, I will immediately update any applicable forms with such information. I understand that I am allowed to remain with my child(ren) for the duration of the activities hosted by Flour Power in order to monitor contact with food.
